Cold Medicines That Actually Work: What to Take for Every Symptom
Learn which ingredient treats which symptom, why some popular decongestants don’t work, how DayQuil and NyQuil compare, and how to avoid doubling up on acetaminophen.
Cold medicines that actually work: what to take for every symptom
The cold and flu aisle holds dozens of boxes, but most of them are built from the same handful of ingredients. Once you know which ingredient treats which symptom, you can skip the guesswork, stop paying for medicine you don’t need, and avoid doubling up by accident.
No medicine cures a cold. Most colds clear on their own in 7 to 10 days. The goal is to feel better while your body does the work.
Match the ingredient to your symptom
| Symptom | Ingredient that helps | Common examples |
|---|---|---|
| Stuffy nose | Pseudoephedrine, or saline spray | Sudafed (behind the counter) |
| Dry, hacking cough | Dextromethorphan (DM) | Delsym, Robitussin DM |
| Chest congestion, thick mucus | Guaifenesin | Mucinex |
| Runny nose, sneezing | Older antihistamines (cause drowsiness) | Chlorpheniramine, nighttime formulas |
| Sore throat | Numbing lozenges or sprays, pain relievers | Benzocaine or menthol lozenges |
| Fever, headache, aches | Acetaminophen or ibuprofen | Tylenol, Advil |
Stuffy nose: the decongestant most people buy doesn’t work
Works
Pseudoephedrine
Kept behind the pharmacy counter. Ask for it and bring a photo ID. It can raise blood pressure and keep you awake.
Doesn’t work
Oral phenylephrine ("PE")
On open shelves in many daytime and multi-symptom pills. The FDA found it no better than a placebo and has proposed removing it.
Decongestant nasal sprays like oxymetazoline (Afrin) work fast, but use them no more than 3 days in a row. Longer use causes rebound congestion that is worse than the cold. Saline sprays and rinses are safe to use as often as you like.
Cough: dry or wet?
Dry cough that keeps you up
Dextromethorphan calms the urge to cough. A 12-hour version can help you sleep through the night.
Wet cough with mucus
Guaifenesin thins mucus so you can clear it. It works best when you drink plenty of water.
Honey
A spoonful before bed eases cough about as well as many syrups. Never give honey to babies under 1.
If you take an antidepressant, check with your pharmacist before using dextromethorphan. Together they can raise serotonin to unsafe levels.
DayQuil vs. NyQuil
| Typical daytime formula | Typical nighttime formula | |
|---|---|---|
| Pain and fever | Acetaminophen | Acetaminophen |
| Cough | Dextromethorphan | Dextromethorphan |
| Extra ingredient | Often phenylephrine | Doxylamine, a sedating antihistamine |
| Watch out for | A decongestant that likely won’t help | Drowsiness into the next morning; no driving |
Formulas vary by version, so read the Drug Facts label on the box you buy. Some nighttime liquids also contain alcohol.
The multi-symptom trap
"Severe" and "all-in-one" products treat symptoms you may not have. Worse, many contain acetaminophen, so adding Tylenol on top can push you past a safe daily dose. Buying single-ingredient products for only the symptoms you have is usually cheaper and safer.
What about vitamin C, zinc, and other remedies?
| Zinc lozenges | May shorten a cold slightly if started within a day of symptoms. Avoid zinc nasal sprays, which have been linked to loss of smell. |
| Vitamin C | Taking it after symptoms start does little for most people. |
| Echinacea | Studies are mixed, and it can interact with some medicines. |
| Rest, fluids, humidifier | Free, safe, and they genuinely help. |
Who needs a different choice
High blood pressure or heart disease
Skip decongestants. Look for products made for high blood pressure, which leave them out, and use saline for stuffiness.
Pregnant or breastfeeding
Ask before taking any cold medicine, especially decongestants in the first trimester.
Glaucoma, enlarged prostate, or thyroid disease
Decongestants and sedating antihistamines can make these worse.
Adults over 65
Sedating nighttime formulas raise the risk of confusion and falls.
Children are different
Don’t give over-the-counter cough and cold medicines to children under 4, and check the label age limits for older kids. For young children, try saline drops, a cool-mist humidifier, and honey if they are over 1. Dose fever reducers by weight using the device that comes with the product.
See a doctor if
- Symptoms last more than 10 days, or improve and then get worse
- A fever lasts more than 3 days or goes above 103°F
- You have trouble breathing, wheezing, or chest pain
- You have severe sinus pain or ear pain
Quick answers
Can I take cold medicine with Tylenol or Advil?
Ibuprofen is usually fine alongside a cold medicine. Before adding Tylenol, check whether your cold medicine already contains acetaminophen.
Do antibiotics help a cold?
No. Colds are caused by viruses, and antibiotics only treat bacteria.
Is it a cold or the flu?
Colds come on slowly and stay mild. Flu hits suddenly with fever, body aches, and exhaustion, and may need an antiviral within 48 hours.
Bring to the pharmacy counter
- Your main symptoms and when they started.
- Any cold or pain products you have already taken today.
- Your prescriptions, especially for blood pressure, depression, or anxiety.
Standing in front of the cold aisle?
Tell your pharmacist your symptoms. They can point you to the one or two ingredients you actually need and make sure they are safe with your health conditions and medicines.
